Hearth's Approach

At Hearth, we offer a long-term home solution to individuals who have Supported Independent Living (SIL) in their NDIS plan. 

Over the past six years, we have developed the Hearth Home solution – a person-centred approach, that combines SIL supports with a home for three participants, purchased by Hearth. It reflects learnings from sector consultations, government engagement and pilot programs, as well as close collaboration with participants, their families and nominees.

Our Solution

If you are a NDIS participant with SIL funding in your plan, you are eligible for the Hearth Home solution, which includes:

 (1) Housemate matching: we facilitate finding two individuals who you may enjoy living with through a thorough matching process. (2) Person-centred home purchase: we buy a house for you and your housemates to live in, informed by your preferences and our home standards. (3) Long-term lease and transition support: you and your two housemates enter a long-term rolling lease agreement with us, and we support you through your transition into your new home.

Our person-centred approach sets us apart: we match housemates first, we buy a house second. Unlike the traditional approach of filling vacancies in existing properties, we start with people.

Funding

Housemates

Finding Participants compatible housemates to live with

Participants can take part in our matching process, which connects people with SIL funding who may enjoy living together. 

Alternatively, participants may already know individuals with SIL funding they’d like to live with.

Coordination

Coordinating funding and roster-of-care with housemates

Hearth coordinates rosters-of-care and confirms funding matches for the housemates.

We assess support ratios to ensure that they are aligned for sustainable, long-term support within the home.

Please note: If NDIS support ratios do not align, living together may not progress.

Buying a home informed by the housemates needs and preferences

Hearth works with housemates and their families or nominees, to choose a home they like.

Confirmation of standard lease agreements and move-in dates. 

Hearth purchases the house. 

We support the housemates through their transition into their new home.

Estimated transition time: 4 months - 12 months
What will your home look like?

Hearth buys a home for you and your housemates, following the matching process. Each home is different, informed by the housemates’ needs and preferences. We also consider key features to ensure the individuals can live their best, independent lives.

How does the matching process work?

The NDIS average SIL support ratio is 1:3 (1 support worker: 3 participants), requiring individuals to typically live with two other individuals with SIL in their NDIS plan. The greatest challenge for participants is finding housemates they may enjoy living with, and that all housemate’s funding aligns to ensure sustainable support. 

Our thorough matching process: We first bring together three compatible individuals who may like to live together, based on preferences of location, housemates, and shared interests. We then coordinate NDIS funding and rosters-of-care to ensure sustainable support for everyone. This process is key and highly complex; if support plans do not align, living together may not be possible.

What are the lease details?

A standard residential lease with a flexible 5+ year rolling agreement for three housemates. It is designed to be a long-term home with the option to move if needed. In your home, you have the freedom to live independently as you like, with no restrictions such as; meals times, activities, appointments or visitations.
