Eva Chan
Eva is the former head of Glenallen Physiotherapy, with over 17 years experience in the role. One of the most skilled physiotherapists in Victoria, she learned about Hearth in 2023.
Eva wanted to join Hearth as she was eager for the opportunity to work together with the passionate Hearth Support Workers to deliver quality and consistent exercise programs in the home and community for people with a disability.
EVA’S SCOPE OF PRACTICE:
Assessment
Motor development and motor function in different age groups
– posture & balance
– Sitting, standing, walking and gait assessment
– Comprehensive Functional Assessment (in conjunction with OT/SP)
– Risk of falling assessment
Assess and prescribe appropriate equipment
– Standing and walking aids
– Seating and positioning
– Wheelchair prescription
Exercise programs:
Improve strength, flexibility, balance and walking ability
Exercise and rehabilitation programs at home or at a gym
Hydrotherapy/aquatic physiotherapy program
Fun and exercise, eg Music and movement; Assistance to access sports program in the community.
Training:
Train Support Worker and family members to carry out exercise program
Training and support for inclusion in mainstream schooling
Manual handling training
