Eva Chan
Eva is the former head of Glenallen Physiotherapy, with over 17 years experience in the role. One of the most skilled physiotherapists in Victoria, she learned about Hearth in 2023.
Eva wanted to join Hearth as she was eager for the opportunity to work together with the passionate Hearth Support Workers to deliver quality and consistent exercise programs in the home and community for people with a disability.
EVA’S SCOPE OF PRACTICE:
Assessment
Motor development and motor function in different age groups;
- Posture & balance
- Sitting, standing, walking and gait assessment
- Comprehensive Functional Assessment (in conjunction with OT/SP)
- Risk of falling assessment
- Assess and prescribe appropriate equipment
- Standing and walking aids
- Seating and positioning
- Wheelchair prescription
Functional mobility and fitness
- Focus on improving and building up your functional mobility and gross motor skills.
- Provide strategies to improve fitness and address medical conditions.
- Exercise programs customised to your individual needs, developed and implemented by our physiotherapist.
- Training provided by our physiotherapist enables support workers to assist you to carry out an exercise program at home, at a local gym, or in a hydrotherapy pool.
- Help you to identify and access relevant sports activities or exercise programs in the community.
